1. Basic information of the accident unit

Luan Group Minmetals is a normal production mine with an approved production capacity of 4.5 million tons/year. Lu 'an Group Jinzhong Branch (hereinafter referred to as Lu 'an Jinzhong Company) is a secondary management company under Lu 'an Group, responsible for the daily safety supervision of Lu 'an Group Minmetals. Shanxi Xinge Construction Engineering Co., LTD. (hereinafter referred to as Xinge Company) was established on July 18, 2003, with a Grade 1 qualification of mine engineering construction general contracting. Shanxi Yachang Technology Co., LTD. (hereinafter referred to as Yachang Company), established in January 2018, has not obtained safety production license and mining construction related qualifications. Yachang company borrowed the qualifications of the new Ge company to participate in the bid to contract the Luan Group five mine main inclined shaft fire extinguishing installation project.

The accident site is located at the transfer point of the second belt conveyor and the first belt conveyor in the 2# main inclined shaft. The 2# main inclined shaft has a inclined length of 952m, a vertical depth of 240m, and the maximum slope of the roadway is 15°. It is a one-track roadway with two belt conveyors and one elevator.

Second, the direct cause of the accident

The site operators placed the bundled steel pipe on the conveyor belt of the belt conveyor, rode on the steel pipe in violation of regulations, and transported the steel pipe to the sliding way by pedaling the conveyor belt with legs. Due to the speed loss of control, the collision with the coal chute and the coal board at the lower end caused an accident.

Three, the accident exposed the main problems

1. Outsourcing unit. First, the new Ge company will illegally lend the construction qualification to Yachang company, which does not have the mining engineering construction qualification. Second, the safety management system of Yachang company is not perfect, there is no safety management organization, there is no relevant management system, and there is no safety management personnel. Labor and employment are not standardized, labor contracts are not signed with construction personnel, and safety production education and training are not carried out.

2. Lu 'an Group Minmetals. First, the entrance control of the construction unit is not strict, the qualification of the project leader and management personnel is not reviewed, and the record of labor employment of the construction personnel is not strictly examined. The second is that the construction unit is not included in the unified management, the daily safety production education and training of construction personnel are not organized, and the project management and control are lost at various levels. Third, the comprehensive regulation of safety production is mere formality, and the hidden dangers of investigation and regulation outsourcing projects are zero; The lack of safety supervision and inspection, the ineffective implementation of the daily anti-" three violations ", the construction site is not equipped with safety supervisors according to the regulations, and the system of check-up of personnel entering the well is not strictly implemented.

3. Parent company. First, Lu 'an Group and Lu 'an Jinzhong company did not seriously implement the safety management responsibility of Lu 'an Group Minmetals, daily supervision and inspection is not strong, the quality is not high, the coverage is not complete, failed to find Lu 'an Group Minmetals safety management chaos, outsourcing engineering out of control, hidden trouble investigation and management is not thorough and other long-term problems. The second is Lu 'an Group and Lu 'an Jinzhong company to implement safety rules and regulations, superior requirements and other gaps, promote video anti-" three violations "progress is slow, not strictly urge Lu 'an Group Minmetals to carry out comprehensive management of production safety. Third, Lu 'an Group is not equipped with the Lu 'an Group minmetals "five chief", resulting in the mine Party secretary long-term acting chief responsibilities.

4. Safety regulatory authorities. Yangquan City Emergency Management Bureau for the "five team", coal mine safety supervision commissioner is not reasonable, coal mine supervision and law enforcement plan does not meet the requirements, the coal mine supervision and law enforcement is not enough, the daily inspection is not strict, failed to find Lu 'an Group Minmetals long-term hidden problems.

4. Investigate and handle the situation

A total of 24 persons responsible for the accident were proposed to be held accountable. Among them, the general manager of Yachang company, the legal representative of Xinge company, Lu 'an Group Minmetals machinery transport area deputy district chief 10 people handed over to judicial organs; Lu 'an Group Minmetals Party secretary, director of safety supervision, mechanical and electrical deputy mining director and other 11 people were given party discipline and administrative penalties and penalties; Lu 'an Jinzhong Company Party secretary, resident mine safety supervisor and other 2 people were given party discipline and government sanctions; The legal representative of Yachang Company shall be transferred to the relevant units for processing.

Lu 'an Group Minmetals fined 5.15 million yuan; Shanxi Provincial Department of Housing and Urban-Rural Development shall revoke the first-level qualification of general contracting for mining engineering construction of Xinge Company according to law; Yangquan City market supervision Administration shall deal with Yachang company according to law.
